On 11/16/2022 at 10:30am, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. Upon arrival, LPA met with Director Josefina Fennessey at the main office located across the preschool and proceeded to tour the preschool classrooms. The following ratios were observed: Sea Treasure room had 11 children supervised by Teachers Lisa Brown and Hiromi Huffman and teacher aide Samantha Armes. P4 room had 14 children supervised by teacher Mary Whiteman and teacher aide Claire Cruess. P4.2 room had 13 children supervised by teacher Elizabeth Underwood and teacher aide Carmel Stralen. Facility is within staffing ratio and all staff members have the required background clearances and are associated to the facility. All required notices, forms and license were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and portable drinking water from an operational water fountain are available both indoor and outdoor. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cots which are kept in the classroom and each cot has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a small kitchen which is clean and free of hazards. Snack menu is posted. Adequate food is available for snacks. Children brings their own lunch to school. Cleaning supplies are not accessible to children. Outdoor play area is a fenced playground with sufficient rubber cushioning. Climbing structures, swings and slides are securely fixed to the ground. appropriate and separated by age groups preschool. Outdoor area has an operational drinking fountain and grounds are free of debris or potential hazards. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the nurse station at the Director's office across the street. Personnel and client records were reviewed, and staff members have the required immunizations and completed the online mandated reporter training. Reporting requirement was reviewed with the Director. LPA reviewed Emergency Disaster Plan and the last fire drill was conducted on 09/28/2022 and the last earthquake drill was conducted on 10/20/2022. Facility has an operating carbon monoxide detector. Facility has at least one staff member that has a valid EMSA approved CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The facility appears to be in substantial compliance. A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.
