On 7/172024 @ 9:31AM Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced visit in reference to the above allegation. LPA met with Director, Erica Rocha. A tour of the facility was conducted with Ms. Rocha. Observed present today were 36 preschool children. LPA conducted interview of staff, made a confidential names list, and received a copy of the children’s roster.
Based on information obtained through interviews with staff and record review LPA determined that the preponderance of evidence has been met. There is enough supporting information to prove the above allegations are SUBSTANTIATED, see Type B deficiencies cited on the attached LIC 9099D.
Type B deficiencies if not corrected poses a potential risk to the health, safety or personal rights of children in care.
LPA reviewed and provided a copy of this report with Site Director. Appeal rights and Notice of site visit were also provided. Notice of Site Visit must be posted for 30 days.

Ms. Rocha will never leave an aide in the classroom by themselves. She will always assigned a fully qualified teacher with the aide.
She will submit her corrections in writing no later than 7/19/24.

Based on the interviews conducted with staff, it was determined that 2 aides were left to supervise preschool children for a few hours on different ocassions.

Ms. Rocha stated that she will obtain professional exterminator company no later than 7/19/2024. She will submit her plan of correction in writing.

Based on interview with staff it was determined that a mice infestation occurred sometime around May or early June. Director did not seek professional exterminators to rid of the mice infestation.
