|Unannounced plan of correction inspection made by Licensing Program Analyst Tulam Vu to follow up on deficiencies cited on 05/09/2017. Met with Elisabeth Hagen, Center Director. Room #2 had 12 children with 2 fully qualified teachers. Room #5 had 16 children with 2 fully qualified teachers. Room #6 had 20 children with 2 fully qualified teachers. The facility operates within licensed capacity and ratio limitations. A review of staff records on 05/18/2017 indicated that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. |
Deficiencies cited on 05/09/2017 have been corrected as follows:
- All parents have been reminded of the sign in and out procedures. A reminder was also typed out and placed on the front of each sign in/out binder. During today's visit, children's sign in and out binders were reviewed showing parent/guardian’s signatures and time of day recorded.
- All children's bedding are placed in each child's cubby. Napping cots were observed individually stored.
- The expired Epi-Pen was given back to the child's parent. The director has implemented new procedures on medication. Medication will be reviewed and updated every three months by the director. New Epi-Pen was inspected today and new procedures were also reviewed.
All deficiencies cited on 05/09/2017 have been cleared.

The Notice of Site Visit (LIC 9213) was provided to be posted at the facility for 30 days. LPA observed form LIC 9213 posted. An exit interview was conducted.
