On 7/18/2024, Licensing Program Analysts (LPA) Adrian Castellon and Julieta Abrego conducted an unannounced annual inspection inspection. During the inspection LPAs observed an incident involving a child in care, C1. This report will be used to memorialize the incident and citation. LPAs discussed the incident with Assistant Director Rosa De La Torre.

As LPAs were touring and inspecting the infant playground and returning to the main office, LPAs observed C1 on the preschool playground wearing only a diaper and without supervision. Child left classroom #9, preschool #1 without the knowledge of classroom staff. The incident occurred at approximately 11:00am.

Per California Code of Regulations, (Title 22, division 12 & Chapter 1) one (1) Type A citation is being cited on the attached LIC 809-D.



LPA Castellon informed Assistant Director that this report dated 7/18/24 documents (1) Type A citation. Notice of Site Visit shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Castellon informed the Assistant Director to provide a copy of this licensing report dated 7/18/24 that documents the Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by child in care left classroom without staff knwledge and was unsupervised for at least one minute. This poses an immediate threat to the health and safety of children in care.
