On 1/29/25 at 10:40 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced complaint visit for the complaint received on 11/8/24, regarding the above allegation. Upon arrival, LPA met with Director Vanessa Milroy and toured the facility. LPA observed a total of 48 children in care with the following ratios:

• Classroom #3 & #4 - 12 children on playground with staff members Isabella Garcia and Sonja Horne
• Classroom #5 - 12 children with staff member Kera Ledbetter
• Classroom #6 - 12 children with staff member Penelope “Penny” Peloquin
• Classroom #8 - 12 children on playground with staff member William Berge
• Classrooms #7, #9 & #10 were not in use

During this visit LPA interviewed three staff members and Director. It is alleged that the facility is operating out of ratio. Based upon documents received from the facility, interviews with reporting party, staff members, enrolled children and parents of enrolled children it is determined through several interviews that approximately once a month, since October 2024, in a couple of the classrooms that there were 13 children with one staff member, which is usually quickly corrected by administration within a few minutes either by moving a child to a different classroom or adding an additional staff member. The allegation is valid because the preponderance of evidence has been met, therefore the allegation is found to be SUBSTANTIATED.

See LIC9099-D for Type B deficiency cited. Exit interview conducted and report was reviewed with facility representative Director Vanessa Milroy. Notice of site visit was provided and must remain posted for 30 days.
