On July 13, 2022 at 9:30 a.m. Licensing Program Analyst (LPA) Leilani Curtis conducted an unannounced inspection to deliver the findings on the complaint allegation referenced above. Upon arrival LPA met with Director Cheryl Gillins and proceeded to tour the facility. There were 7 children present with 3 staff members. Appropriate ratio/capacity was observed. Staff members have the required background clearances and are associated to the facility.

The initial complaint investigation was conducted by LPA Curtis on 5/4/22. Throughout the course of investigation, interviews were conducted with several employees and several parents. Facility records were obtained and reviewed. The information gathered indicates that on 4/18/22 five children were allowed to remain at the facility with symptoms of illness including but not limited to runny noses and/or congestion.

Based on interviews conducted by LPA and a record review the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED, California Code of Regulations, Title 22, 101226.1(a) is being cited on the attached LIC 9099D.

The director states that she will conduct a staff meeting regarding the daily inspection for illness of children and when children are allowed to be accepted into care. She will discuss what symptoms of illness to look for and how to document their findings. The director will send LPA a copy of the meeting agenda and staff sign in sheets via email by POC due date of 8/12/22.

An exit interview was conducted with Director Gillins and appeal rights (LIC 9058 1/16) were discussed. A copy of this report as well as a copy of the appeal rights were given to Director Gillins. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post Notice of Site Visit.
