As a result, one Type B deficiency is being cited today under California Code of Regulations, Title 22, Division 12 on the attached LIC 9099D. This report must be made available to the authorized representatives
of children in care and maintained in the facility file for three years.
A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the
main door for 30 days.
