|Licensing Program Analyst (LPA), Rene Mancinas JR, conducted an unannounced case management inspection to follow up on an unusual incident that occurred on 05/18/2018. LPA met with Site-Supervisor, Linda Coffman. LPA took a census, toured the facility, reviewed staff statements and facility records, and interviewed staff regarding the incident. |
During today’s inspection, it was determined that on 05/18/2018 at approximately 05:00pm, Child #1 was left unattended on the outdoor playground without any supervision. The playground area is completely fenced in by means of a chain-linked fenced. Children had just transitioned from the playground to inside the classroom. A passerby, contacted Site-Supervisor, Linda, to inform her of Child #1 being outdoors alone in the playground area. Linda checked the outdoor playground area and located Child #1 alone. Child #1 was taken to their assigned classroom and parent was notified. Linda reported the incident to Licensing. Linda also stated she was going to implement a new training session for all staff on the protocol for exiting and entering the classroom while ensuring all children are accounted for. Linda has been monitoring staff conduct extensive yard searches since incident occurred until formal staff meeting takes place on or before 06/01/2018.
Per California Code of Regulations Title 22, Division 12, Chapter 1, the following deficiency is being cited. See (809-D) deficiency page.
Upon receipt of a Type A deficiency, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. The licensee was provided a copy of the “Fact Sheet” for AB 633 (Parent Notification Requirements). Licensee must have all parents of enrolled children sign the Acknowledgement of Receipt of Licensing Reports and must retain a copy in each child’s file.
An exit interview was conducted with Site-Supervisor, Linda Coffman, and appeal rights were provided. Notice of Site Visit to be posted for 30 days. This report shall be made available to the public upon request.
