|Licensing Program Analysts (LPAs), Kathie Campbell and Norma Lomeli conducted an unannounced case management visit to follow up on an unusual incident report that occurred and was reported on 3/7/16. LPA's toured the facility, interviewed staff and reviewed files. The facility is located off of Mooney Blvd and there is a shared road between the facility and the Tulare County Government Center. There is a canal running along the north side of the center. The center has chain link fencing around the perimeter of the playground. During the visit it was determined that a child wandered off from the facility by crawling underneath a fence on the north side of the playground that had a gap large enough for a child to fit through. Child #1 crossed the shared road between the preschool and the Tulare County Government Plaza and was sitting down at a table in front of the building. Staff didn't realize the child #1 was missing until the parent came to pick the child up. A security guard at the Government Center saw staff looking for the child and pointed out where the child was. After returning to the center the child showed the parent how the he was able to get out. Staff was aware of the gap in the fence and reported their concern to the previous site supervisor. Staff stated they don't check the playground prior to the children going outside. Ms. Pettitt stated she did walk the perimeter of the playground prior to this incident but she did see that fence but she didn't notice the gap was that big.|
Per Chapter 1, Division 12, Title 22 of California Code of Regulations the following deficiency was cited today. See (809-D) on next page.
Exit interview was conducted with site supervisor, Kwanda Pettitt. Site supervisor, Kwanda Pettitt was provided a copy of their appeal rights (LIC9058 12/15) and their signature on this form acknowledges receipt of this form. LPA observed the Notice of Site Visit report was posted and advised to post for 30 days where parents may easily view.
"Upon receipt of any A violation, licensee shall post and provide copies of this licensing report and LIC9224 to parents/guardians of children in care at the facility and to parents/guardians of children upon enrollment at the facility during the next 12 months, LIC9224 to be signed verifying receipt and filed in children's files."
