|Licensing Program Analyst (LPA) Malek visited the above facility on this date to conduct the above complaint investigation. LPA met with director, Beatrice Torres and Renee Huber in the office at the time of the visit. There were a total of 22 infants and toddlers with 7 staff in 3 different rooms observed. |
LPA reviewed the time documents, sing in/out sheets and interviewed the staff regarding the above allegations. The staff stated there were always enough staff in the infant room to cover the crib area after the last citation. The staff denied being out of ratio in the infant room. The staff or individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. Based on the information gathered, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is inconclusive. Notice of Site Visit was posted.
The notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The licensee was provided a copy of their appeal right (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.
